SUMMARY:

The Quality Consultant will work with providers and stakeholders across the care continuum to implement evidence-based interventions aimed at improving health outcomes and other quality improvement areas of focus as assigned.? The consultant will assist with program implementation and facilitation of partner collaborative meetings, disseminate project information to healthcare providers, stakeholders and partners and assist with the spread of best practices.

DUTIES AND RESPONSIBILITIES:???

  • Actively participates in?a collaborative team model to identify strategies and interventions across?the healthcare continuum
  • Utilizes a collaborate?approach to apply quality improvement knowledge and acquired technical expertise and experience in supporting the assigned?project
  • Assists healthcare?providers to progress with measures reporting through the development and implementation of effective strategies and interventions
  • Assumes designation on?interventions as assigned by the manager
  • Develops and monitors?timelines and workplans relevant to assigned tasks
  • Accountable for meeting project deadlines and deliverables
  • Models accountability for self-learning activities as well as the sharing and transfer of knowledge?gained
  • Assists with contract?program planning and implementation
  • Develops and maintains?relationships with external customers primarily task/contract?participants
  • Adheres to all relevant?compliance regulations (HIPAA, FISMA, URAC, CMS)

EDUCATION AND/OR EXPERIENCE:?

  • Bachelor’s Degree in healthcare related field is required
  • CMS QIO experience a plus
  • Five (5) years of healthcare?experience is preferred; RN preferred but not required
  • Excellent communication?and facilitation skills required
  • Quality improvement?experience including process and program development preferred, Certified Professional in Healthcare Quality (CPHQ) preferred but not required
  • Experience in social determinants and health equity a plus.
  • Previous experience leading projects is preferred
  • Excellent customer service skills required
  • Ability to handle multiple tasks in a fast-paced environment
  • Proficient in the Windows environment with experience in the following software: Microsoft Word, Microsoft Excel, Microsoft PowerPoint, Microsoft Teams
